I have not received my baby's birth certificate. Can Valley Presbyterian Hospital send this to me?
Valley Presbyterian Hospital obtained the information from you during your maternity stay to populate the Birth Certificate. The actual Birth Certificate comes from the Los Angeles County Department of Public Health, Office of Vital Records.
Requests for Birth Certificates are available through the Office of Vital Records in person, by mail, or through the Vital Records website. Please visit the website link below for fees and forms that are needed for your request. Contact the Los Angeles County Registrar-Recorder/ County Clerk for additional information.

In order to get the Social Security number, you will need to contact the United States Social Security Administration. The closest office to the hospital is:
