Admission Process
Valley Presbyterian Hospital is committed to providing an easy and streamlined patient admitting process. To expedite and minimize your wait time on the day of admission for a prearranged procedure or surgery, please follow the steps below before you are admitted. Step 1 - Your Physician Schedules Your Procedure or Surgery
Your physician will arrange for your admission to Valley Presbyterian Hospital (VPH) for your procedure or surgery, and may also request that you have preoperative tests performed in advance of it either at his/her clinic, another lab, or at VPH. These appointments are scheduled 2 to 14 days before your admission. If you are an Obstetrics (OB) patient having your baby at VPH, please scroll down to the bottom of the page for special Admission information.
If you have questions or concerns regarding your preoperative test instructions, procedure or surgery, please notify your pre-op nurse or surgeon before the day it is scheduled.

Step 2 - Call or Visit Our Scheduling Office to Preregister
Once you have been scheduled or referred to the hospital, please preregister by calling or visiting the Valley Presbyterian Hospital Scheduling Office at least three (3) business days before a preoperative test appointment or scheduled procedure/surgery. If your physician requested preoperative testing and you do not complete it, or we haven't heard from you, you may be contacted by a patient representative.

Please be prepared to provide the following information before calling or visiting, as it applies to you:
- Orders given to you by your physician's office.
- Date of your scheduled procedure.
- A valid, unexpired government-issued identification card with your photo. We will also need your current address, telephone number, and name/address of your employer if you have this information.
- Social Security numbers. Social Security information is often required for patient and spouse.

Health insurance plan. If you have one, bring the name of your plan and insurance identification
card(s), including your prescription drug card. If a pre-authorization
from your insurance company for the surgery or procedure was required,
please bring a copy of it.
- Medicare patients. If you are a Medicare patient, bring your identification card and be prepared to provide retirement dates for patient and spouse.
- Pediatric admission. If a child is being admitted, bring the parent's insurance identification card and be prepared to provide birth dates and social security numbers of patient and parents.
- Payment for out of pocket costs. If you have an out of pocket cost, you will be required to provide a form of payment before your procedure or surgery. All forms of payment are accepted (cash, check or credit card).
- A list of medications and allergies. Make a list of prescription and over-the-counter medications, and any herbal medicines, vitamins, and supplements. Note how often you take them and how long you have been taking them. Also include a list of allergies, including any foods, insects or any other product that may cause an allergic reaction.
- AICD or pacemaker card. Either bring the original or a copy that includes the front and back.
- Laboratory reports, and other required reports or documents. See Step 3 below.
Step 3 – Forms to Fill Out Before Your Admission
You will also be asked to provide signed forms before we can complete your admission. To expedite the process, you have the option of downloading and pre-signing these documents and bringing them with you on your day of admission. These documents include:

- Special consent forms. If you are having surgery, you also will be asked to sign a special consent for your surgery and the administration of anesthetics during the admission process. If you are a parent for a minor or a legal guardian, you must sign these consent forms.
Step 4 – Sign Up for the Patient Portal
As an optional convenience, Valley Presbyterian Hospital has an interactive Patient Portal to securely access your health information online from any device and share the information with your healthcare team or designated caregivers.
We encourage all patients to enroll and actively participate in their healthcare. Once enrolled, you can view laboratory and radiology reports, medications, allergies, demographic information, visit history and discharge/aftercare information.

Step 5 – How To Prepare, and What To Bring and Expect on Your Admission Day
For important information on how to prepare for your procedure or surgery (what to eat the night before, what to wear, what to bring to the hospital, requirement for someone to drive you home afterward, etc.), please visit our Preparing for Your Stay page.
On your admission day, please arrive two (2) hours before your scheduled procedure or surgery. Check in at the Main Lobby Information Desk located on the first floor where you will be routed to the appropriate registration area. Our Patient Access Services staff will greet you and finalize your registration. If you preregistered and provided all required documents, you only need to bring your identification, and AICD or pacemaker cards. If you have not already preregistered, bring the documents listed in Step 2 above, as it applies to you. If you don't have the documents in Step 3 filled out and signed, you will be asked to complete them when you arrive.
After you provide all required documents, our Admitting staff will give you a letter explaining that any physicians' fees will be billed separately from the hospital bill. Once the admission process is completed, you will then be escorted from the waiting area. Obstetrics (OB) Admission
Obstetrics patients are encouraged to preregister by the sixth month of pregnancy. To request an Obstetrics preregistration packet, please call 818.902.5246.
